I may be wrong but I think Steve (Mercman) reported problems with how Audirvana Plus manages the library. This is a feature that didn't exist in early releases of Audirvana Plus, and a feature you can choose not to use in current versions.Are you in fact trying to use Audirvana Plus to manage your library?
Unless I have my terminology wrong, Audirvana Plus in "iTunes Integrated Mode" continues to use iTunes for library management and you continue to use iTunes as your interface into your music library. Audirvana Plus still launches but you don't really interact with it. It's there to bypass Core Audio, provide sample rate switching, memory play, disable unneeded processes, and a number of other things. In my case I often have Audirvana Plus hidden behind the iTunes window.
I have not tried the library management feature of Audirvana Plus based on what I recall reading in Steve's (Mercman) initial findings. I use it only in "iTunes Integrated Mode" with iTunes managing my library. I haven't had any problems like you describe.
