If nobody else responds here, I'd continue to do internet searches for images of the PAM-1, and hopefully you'll get lucky. And if you can't find a picture of the trace side, it shouldn't be too difficult to hardwire, simply based on the component positioning shown above. Simply connect the leads of the components per the PAM-1 schematic.
