So far here's what I've found: output resistors off by more than 10%.

Other resistors off by more than 20%

2 extra 500 Volt 22uf capacitors around the quad capacitor

Input terminals that are not wired correctly.

Two Questions: What is an acceptable tolerance for resistors?
Why the extra capacitors?
