Mistakes like this happen in the bindery, where the printed 16- and 32-page
printed signatures are folded, cut, and bound. The printer usually has
someone standing by inspecting the output, but if he or she missed it,
there might have been a lot of issues bound with a missing or duplicated
signature.
I am sure that if you contact The Absolute Sound, they will send you a
replacement free of charge, which is what we do at Stereophile when
errors like this happen.
