Foreplay III transformer change to CT

I would like to rebuild my Foreplay III with a new transformer. I would like to use a small Hammond that has a center tap. The wiring seemed simple and I have done this in the past with my FP II. Somewhere along the line, I lost my diagram and also forgot how the wiring change is made. Would anyone be willing to share this information with me ? I realize it is not complicated, but I am just not remembering well enough.
