I've had the same experience and unsuccessfully tried all the fixes you mentioned. I can't tell you why although it seems to be a tubes meets solid state thing, but I can tell you what fixed it for me. I purchased a Jensen ISOMAX isolation transformer. It comes in both mono and stereo versions. Got rid of the hum. This is a link to the product at a good price:
