Seduction Problems - Bad Tube?

I have assembled a seduction kit with the C4S upgrade, and the resistance measurements all seem to check out. The voltages are off, I am only getting about 4.5Vdc on the heaters, and around 140 where I should be getting 160. The other voltage are similarily low. The two leds that are connected to each socket light up. One of these leds however seems to take longer to light up than the rest and when it does it is dimmer. Two of the 4 leds on the C4S board remain dark. They are the upper two as seem in this picture stolen from wardsewb. http://wardsweb.org/audio/seduction/C4S/c4s15.html

If I switch the tubes in the sockets, the dim slow starting led follows the tube, however the two leds on the C4S board remain dark. Any ideas on where to go from here?
