Hi CK,
How long has the DAC been in opperation? Does it get better or worse the longer you leave it on?

I would double check your solder joints on the power supply board (a cold solder joint can cause this) and make sure your regulators are secure. You can probably rule out the analog board as you mention is was both channels.
