Yes, you could do this, but I think that it would be better to provide a separate headphone amp in your integrated if you use headphones very often. There are many solid state and tube designs out there from simple to complex that will do the job with varying degrees of sonic success. I have designed two tube designs in the past year that sounded pretty nice.If you wanted to use a transformer, you would put the appropriate load resistor (4 or 8 ohms)across the primary of a stepdown transformer such as the LL1660/PP connected as alternative Q from the datasheet(with the primary center tap NOT connected to a supply voltage source). Any small DC offset at the output of the amp will not present a problem with this transformer and it will drive 32 to 600 ohm headphones just fine.
