The connection on the amp end is straightforward and as you have it. You should not connect either one of the signal leads to the amplifier audio ground. On the preamp end I would change your output transformer wiring. Take a look at the schematic of my phono stage to see how I advise wiring transformer outputs. Some advocate the RC network from Pin 1 of the XLR to chassis; others advise a direct connection. I use a switch to choose what results in the lowest noise.
