Hard to be sure, could be either or could be a bad batch of capacitors (the same problem many large computer and related gear manufacturers had).

This does not good. It seems the Cap got seriously hot, hotter than the mains transformer.
I suggest you take some pictures and send them to Brian, he or Reckhorn may be able/willing to replace the bad parts.
