How do you prevent the tube from rattling around inside the plastic bottle?

You need to have foam strip at the bottom so that the top engages and holds the tube. Add until it holds perfectly, better than any box.

Easily done. All of my 9-pin signal tubes are shipped inside a small zip-lock bag with a tests slip. When the tube and slip in the bag are rolled up and inserted in a 2-1/2" Rx vial along with a sliver of foam on one side...it makes for a nice fit. No top needed. I didn't get tops with my vials....which was fine...don't need 'em. I might cap it off the vial with a small piece of foam.
