I have had this thing for years and endless research has gotten me nowhere. Maybe some old timer or someone else recognizes this thing. that's four leads out of the bottom, one off to the side.
It appears the lead on the side is for plate voltage. There is a grid. It looks very reminiscent of the Western Electric 205 structurally.
So, any guesses?

My guess is experimental electrometer tube, made by western electric (definitely). The reason it has a long shaft that is tipped off, is they would open and weld the glass into a test rig. It is used for measuring miniscule currents, I believe. More research on electromter tubes will yield some info. Definitely a very early example!

Based on this photo, I believe it's a phototube for optical film projector soundtracks. It's a very early one. I have some later ones, I'll post a photo tomorrow.

I can see all the parts. My pictures stink, but I am very familiar with those photo tubes you posted and this is nothing like it. This has an internal structure very similar to the Western Electric 205D.

Here are a few from my collection. I believe what you have is a variant of these. I'm not sure why yours has that many connections, though.
