most of your requirements are met with the M-Audio Audiophile. I've been using this little box for a couple of years now and have had excellent results with it.

One thing I would recommend you avoid, if possible, is the powering of your outboard sound card via the USB port. Instead, make sure the device you'll use has its own power supply. This eliminates one clear source of noise since most PC power supplies were never designed for dedicated audio applications.
