The mid-bass XO is indeed the main culprit.
You can check Neo's spreadsheet for the 3A XO and look at group delay for each driver at the frequencies around the XO and see the differences. compare them to the driver distances from the listener for each driver.
Your toe in is pretty typical for tweters out and is likely within the design assumptions for placement.
The drivers in the single plane arrangement will have different distances from the listener. You can measure them and translate the differences into time delays and see how they match up with the group delays.
For my own T IV/Neo8 in the equidistant arc setup I did a symmetrical 1st order XO. and I did the same with 3rd Butterworth and 4th order (with my Marchand with the 4th order setup the equidistant arc was the only way things "snapped together")
