I was in Rome 2011 and went to at least 5 different restaurants across town and none had ever heard of pepperoni. And not just in passing, ie even after explaining exactly what a pepperoni was..including our hotel (Hilton at the airport).

Some regions probably do have things like "pepperoni". If not the Italian mainland then Sicily, perhaps.
This was a few decades ago, but I seem to recall that Florence was the place with the weirdest or most exotic looking pizzas. Large deli cases full of various pizzas topped with almost everything imaginable on display.
this is what one restaurant brought us after 4-5 attempts to explaining what a pepperoni was. No worries, it was great nonetheless!
It tastes good so who cares really?
