You are unwise to run the deck at 45 - as it will stretch the belt - this causes some problems when used at 33...Get another belt - use this one for 45 if you use that speed frequently..
The belt should sit on the 'crown' of the pulley.
In any event, what you're describing is likely to be attributable to the 'lifting lever' not doing its job properly...so maybe it's slightly misaligned/bent/malformed and unable to 'heave' the belt up any further..
..although there is a possibility that the motor itself may be incorrectly angled (this also affects the speed - you should check with a stroboscope..always a good idea).
