The middle filler piece sits inside the larger piece. It is balanced by a small brass protrusion inside and in the center of the larger piece. The smaller piece has a small indentation on its bottom to center this protrusion, allowing the smaller piece to move slightly. The hole in the small piece is threaded, but has room for a small spike without hurting the threads.
That's the best i can do. I don't recall the price. I bought 8 of these many years ago. It was well under $500.
I can have them made in a machine shop, but it won't be cheap.
