Also, to the original poster, if Bob Latino's right about your rectifier tube being the cause, it would be wise to follow the advice Roy Mottram posted on the VTA forum. (link below)

I'm not suggesting the diode mod could have prevented your failure, but it's cheap insurance for the less than robust quality of today's 5AR4 tubes.
