Just wire the heater lines to #1 and #9 --- in the 5670. And you are using 6.3VAC (of a 6.3VAC PT heater tap).
Wiring the 12AX7, #4 & #5 (connected together) to one line and the other to #9 = 12.6 VAC in parallel.
All off the same heater taps.
