Genalex Gold Lion 6922

During the past year, I purchased twelve of these from a reputable tube vendor who provides some level of break-in and sorting. In the first 100 hours I lost two due to excessive noise. At around 200 hours I lost another to noise. Now at about 400 hours, I lost a forth. In each case only one triode went bad.

These tubes are used in a Cole LCR phono stage and a Herron VTSP-2 line stage. Neither of these components run the tubes hard.

Has anyone experienced this? Any suggestions? I like the sound of these tubes but don't want to keep throwing money ($45 a pop) at tubes that are likely to continue failing prematurely.
