Help - EL34 dead

Hello - your guidance will be appreciated by an ignorant user.
I have an Icon audio Stereo 40 integrated amp. 1 pair 12AT7, 1 pair 12AX7, and a quad of EL34s. All are EH.
Yesterday, after playing 3 seconds of music I have noticed that one of the 34s doesn't light up (bias voltage is Zero). I'll do some swapping today - but the question is - other than just a malfunctioning EL34, could it be that some other tube is making the problem? I did notice that one of the smaller tubes' top of the glass is black, whereas all others are silver on top.
