I used these on my power supplies but now I cannot recall their model designation. IIRC, they were made by Amphenol. You might try emailing the pix to http://www.amphenol.com/index.cfm or http://www.peigenesis.com/amphenol.html and see if they can identify them. They were pretty standard a decade or two ago.
