Hello
Can the LL5121 (very similar to LL1540) be used for fase splitting at the input of amp . I know it is no longer produced but I have a couple of them which a want to use . If so what are the connections when used unbalanced in . unbalanced out .

You can use it for phase splitting by using it 2.28:1+1. It will perform better with respect to common mode rejection with resistor loading like that shown on the LL1540 data sheet.
