Lundahl website http://www.lundahl.se/product_process/shortform_list/short_form_list.html describes the LL 1621 as follows:

LL1621 Non-inverting interstage transformer Grid choke

How does one use it as a grid choke? Just use the primaries and leave the secondaries open?

The normal use of that transformer is to provide a sort of hybrid interstage coupling, where the high frequencies are capacitively coupled and the low frequencies are magnetically coupled. One could use the secondary(s) as an input choke with an open primary(s), but I would use the LL1670 as a grid choke instead. Much higher inductance and smaller size makes it a better choice.
