 I'm looking for a 120v AC to 12v DC converter with a 1/4 inch RCA output plug. Any help would be appreciated

 check Mouser or Digikey. Theyn have LOTS of 12VDC "wall warts". However, they may NOT come with a phone plug, which you would have to order separately and attach yourself.

You didn't specify current draw. Is filtered DC OK? Does it have to be regulated?

 Not regulated; 1.5 amp from wall; 5 to 6 amp out

 You might also try a couple of the electronics parts houses, like Altex Electronics. Or a more consumer-oriented place like Batteries Plus might have it. Also try hifi manufacturers/repair depots - they might have a model intended for their products which will also fit your needs.
