Philips had factories in England and in Holland and many other places.
The tubes made in England were normally labeled Mullard. But not always.
The tubes made in Holland were normally labeled Amperex. But not always.
Tubes made in any of the Philips plants could be labeled anything. Mullard, Valvo, Amperex, Siemens, Miniwatt, Mazda and others.
The codes tell you which of the Philips factories the tubes were made in.

So, that leaves the question of what factory is represented by 5?
One website says Toshiba, Tokyo, Japan.

I am familiar with audiotubes.com reference. If you scroll further down on that page, there is this link to an 8 page document:

The first page has a similar list and attributes factory codes 1-4, 6-8 to Philips Eindhoven, with NO entry for 5.
It seems likely that this tube was not made in Japan. So Dutch origin, but still no definitive factory attribution.
