Q. What is the philosophy behind active studio monitor volume controls rotation?Many volume controls run flat out at zero and as you reduce the volume show -10, -20 on down to -50. My McIntosh decreases turned to the left, flat straight up, boosted turned to the right. But on the Yamaha HS 50M volume control, all the way counter clockwise shows "min," straight up shows +4db, and all the way clockwise shows -10db. Where is flat?

"Where is flat?"
There is no flat - you need the amplifier that's inside the box to drive the monitor. If you set the volume control so the amp puts out exactly the same signal level as its input, you're not going to hear anything (or maybe, if you hold your ear real close, the barest hint of a whisper.)

Should I run them like McIntosh amplifiers, and just open them up all the way (-10), all of the time, and control the volume with the pre-amp?

It doesn't matter - you've got two pots in circuit, and either one can be used. Use the one you can reach easiest.

+4 is meant to be used if you feed it a pro-level balanced signal (2.83V for 0dB);
-10 if you feed it a consumer level single ended signal (0.7V for 0dB).

What is being fed in coming from a McIntosh pre-amp with RCA output, to XLR input on the monitor?

So it's an unbalanced signal and luckily you have pots and not just switches.
This means that your starting point is the -10dB setting.
All you need to do now is find a point around there at which you can make sensible use of the McIntoshs volume control but keep the noise level as low as possible.
