Hi Jim,
I found some pictures on the web and I think it might be an Onetics transformer. You could try Bud Purvine and see if he can help you.

The link has the pictures I found. I think George used transformers from Onetics for some of his designs.

Bud posts on DIYaudio, so you might be able to contact him there.

I also have an old article written by George and a handwritten schematic that might help. Send me an email and I can return the info to you.
