We had to suspend operations with UPS some years back on account (if you will pardon the expression) of this sort of thing going on all the time.
Whenever we ran into trouble, it would usually be some sort of small item shipped from one place I never heard of to another, not originating or arriving here at all. But it would be on the bill. We found, after an audit, that this was happening on average once every 5 weeks.
We calculated that on average, over a period of 6 months UPS had made off with about $200.00 and had been doing so for years. I did wonder at the time how many other accounts were getting duped in the same manner. So we suspended operations with UPS and went with Fed Ex, who has yet to make such a 'billing error'.
Since your shipment was entirely in the UPS system until it arrived, its unlikely that anyone outside the system was involved, though not impossible.
