Each situation is different but you are correct, surgery is usually needed on the end piece. However, you want to create a solution that doesn't do away with the strain relief. The whole business can be covered by a stout, adhesive-lined heat shrink.
The other thing some, such as Purist, have done with really large ICs is to create a size reduction into a smaller diameter just ahead of the termination. I believe they have done this with some of their cables that are filled with goop, metallic dust, etc.
Another consideration is that you may want to try a different connector. Here is an example of a larger cable into some Furutech XLRs.
